Client Intake Form: The Questions to Ask Before Every Job
What a service business client intake form should include so you capture the right job details on the first call.
Why intake matters
Most return trips and pricing surprises start with missing details from the first call. A consistent client intake form helps whoever answers the phone ask the same questions every time.
What to include
- Contact name, phone, email, and best time to reach them
- Service address and access notes (gate codes, pets, parking)
- What they need done, in their own words
- Photos or measurements, if available
- Preferred dates and how urgent it is
- How they heard about you
Keep it short
An intake form that takes 20 minutes won't get used. Ask only what you need to quote and schedule the job. Then collect the rest on site.
